Procurement strategy for Support at Home
(homecare) services for residents in Hammersmith & Fulham who
have assessed eligible needs. Support at Home aligns with the
administrations manifesto commitments and the council’s
independent living strategy, working compassionately with residents
to provide quality care and support, allowing people to remain in
their home and local community for as long as possible. The focus
of the new procured services is to have better continuity of care
by having carers who are consistent and well trained, a more
Outcome Focused Service, giving residents more control and
flexibility on how their care and support is provided, and enabling
independent living for residents with a strength based
approach.
