Our ambition is to work collaboratively to
support children, young people and their families at the earliest
opportunity. It is imperative for this approach to develop a
strategy of partnership working, early intervention and inclusion
to reshape the way in which we plan, implement and deliver services
in Hammersmith & Fulham in collaboration with our partners and
community and third sector providers. The 0-19 Early Intervention
Strategy will draw on these opportunities to transform outcomes for
children, young people and families in Hammersmith &
Fulham.
