Purpose:

The CCG and Council have a joint duty through the Health and Wellbeing Board to produce a plan that promotes integration and says how it will improve the health and wellbeing of people in its area. The strategy is a five year plan based on evidence of local need and informed by the views of the public, patients and health and care professionals via consultation and engagement. Cabinet are asked to approve a high level plan which sets out a vision for the borough and the health and wellbeing priorities for the CCG and Council over the next five years.

Decision:

That the Hammersmith and Fulham’s Joint Health and Wellbeing Strategy 2016-21 which has been endorsed by the Health and Wellbeing Board and the Health, Adult Social Care and Social Inclusion Policy and Accountability Committee, be approved.
